About
2 minutes after take-off from runway 10 at Eduardo Gomez International
Airport, a loud bang was heared in the cockpit, simultaneously the pilots
received abnormal engine indications from engine no 2 (center, tail). The pilots shut down the engine
(General
Electric CF-6-50C2)
and elected to continue the flight. At the same time, large engine debris
was falling into the densely polulated neighborhood in northern Manaus. 22
houses and cars sustained damage from falling engine parts.
Fortunately no injuries were reported. The DC10 later made a safe landing
at Bogota Airport.

meters coming to a stop halfway down an embankment. All passengers and
crew were safely evacuated. The Embraer suffered a broken nosegear

After
a long take-off run on runway 34, the aircraft experienced tailstrike before the pilots were
able to gain height and a stable climb-out was achieved. The flight was
cancelled and fuel was dumped for a return to Melbourne. On landing runway
16, the A340 hit the ground hard causing further damage but the aircraft
was able to taxy on its own power to the gate. The runway was closed for
inspections and debris collection. No injuries among passengers and
crewmembers were reported. Damage to the tailskid assembly is considered
as substantial. At the time of the incident local weather was calm and
night visibility was good. According to airport officials, a number of
runway edge lights were also damaged or destroyed. Furthermore, it is
understood the A340 became airborne beyond the end of the runway where its
undercarriage took damage after collision with the localizer antenna. It
was also reported, the tail missed a brick building by only 50 centimeters
(!).
NOTE: in April 2004, an Airbus A340-300 (A6-ERN) of Emirates Airline suffered
damage after it collided with approach lights after it became airborne
past the runway end at Johannesburg, S.-Africa. |

final approach to Legazpi Airport, the pilots noticed that a van was
driving alongside the runway and initiated an immediate go-around meneuvre.
The aircraft then landed safely after its 2nd approach. Tower personnel
reported the ATR came very close to the van until it managed to climb away.
On board the van was the son of the airport director and his girl friend
whos he was giving driving lessons on the runway. According to local
media, the windows of the van were closed and the couple was listening to
music preventing them of realizing the approaching aircraft. The airport
director meanwhile was suspended. |

aircraft made a safe return to Frankfurt Rhein-Main International Airport
after an engine failed during the climb-out. Local sources indicated a
fire also erupted briefly in the engine but was extinguished well before
landing.
Note: The incident marks the 2nd of three such occurences within the
month of march after the same aircraft experienced birdstrike resulting in
engine failure (+fire) on approach to Frankfurt on Mar.08. On Mar.28th,
the same aircraft as above was making its first fligth after maintenance
from Frankfurt to Windhoek when again an engine (starboard outer, # 4)
failed forcing the pilots to return to FRA. |

737 was on final approach to runway 35R(right) at Mohammed 5th
International Airport at Casablanca when the pilots erroneousy line-up on
the parallel taxyway instead of the runway. At this time, four other
aircraft were on that taxyway waiting to departure when the error was
detected by one of the pilots who realized, the 737 was aiming directly at
him. After he issued an immediate warning, the error was detected. The 737
executed a safe missed approach and made a safe landing on the correct
runway. At the time of the approach, visibility at the airport was reduced
to about 6 kilometers. |

90 minutes of troubleshooting and unsucessful attempts to lower it, the
aircraft made a belly landing on runway 18 (lengh = 3.353 meters). The
Metro came to a safe stop on the runway. There was no fire.
NOTE: A day earlier, the same aircraft experienced an unsafe gear
indication while on approach to Winnipeg. At this time, the gear could be
extended porperly.

the late stages of the cruiseflight, - whilst flying over Kelowna at 38.000
feet (FL 380) - the pilots reported a power loss in an engine. The A330
continued to fly at a lower than normal altitude and made a safe landing
at Seattle. |
