On landing
Butembo in bad weather conditions, the aircraft drifted off the runway
where its left maingear collapsed and the left wing struck the ground
before the aircraft slid to a halt on uneven, downsloping terrain. None of the
passengers or crewmembers was seriously injured. The Let 410 received
damage to its nose and left maingear as well as the left propeller and
left wing.

The presidential aircraft was on final approach when
it undershot the runway and contacted uneven terrain. The aircraft veered
off the where it lost its entire undercarriage and both propellers failed
before it came to rest about 200 meters further. Its starboard propeller
broke off and high engergy debris impacted the adjacent fuselage section
causing substantial damage. All on board were able to get out without
serious injury. There was no fire. On board was
President Idriss Déby accompanied by his delegation to attend a
development forum at Kalait, all of them remained unhurt. Abnother
source reported the aircraft received irrepairable damage. Bir Kalait
aerodrome only has very limited infrastructure to handle passenger
aircraft. It is believed the aircraft was damaged beyond repair.

to the compartment structure. It is understood that there was no fire and

Immediately
after becoming airborne from runway 34L at Narita airport, the crew
noticed an asymmetric flap retraction indication resulting in a flap
disability. The pilots discontinued the climb-out at a lower altitude and
made a successful 2nd landing attempt at Narita, following an aborted 1st
approach to runway 34L because of a shift in wind direction. The landing
run was made with excessive causing eleven tires to automatically deflate
becasue of intense braking heat. There were no injuries among passengers

After lenghy
engine run-ups on runway 24R, the An-124 began its take-off run without
clearance from the tower. Considering the last communication exchange the
tower previously instructed the pilots to "report when ready for
rolling", the pilot mistook these words as "report when rolling"
and released brakes after take-off power was set. The premature departure
of the Antonov forced the tower controller to turn an Air Canada Airbus
A320 immediately to the south to avoid a possible conflict in the
departure sector. The An-124 made a normal take-off and proceeded normally
to its destination.

In the early
stages of the cruiseflight at 32.000 ft, about 150 NM north of Lusaka, the
left engine #1 suffered an uncontained failure. Two large holes on the inboard side of the engine were visible. Smoke and sparks were
seen coming out of
the engine. The pilots shut the engine down and returned to Lusaka where
the flight landed safely.

Whilst the A330 was pushed
back on a cargo ramp to be towed to position "Y04", one of its
wings hit the neighbouring 777 resulting in damage to both aircraft. Both
aircraft were empty at the time and no one on the ground was hurt. While
the 777 was able to resume operations on Oct 23rd, the A330 remained
grounded for repairs.

On landing
approach to runway 21 at Zweibruecken Airport the aircraft came into close
proximity to an ultralight whilst flying at an altitude of 4000 ft. The
pilots were forced to execute an immediate descend to avoid a collision.
According to unconfirmed reports, the the ultralight passed only 100 ft
above the A320 which was able to complete the approach and a normal
landing was made.

runway 15 rain and thunderstorm activity, the pilots were unable to stop
the 737 before the end of the runway (Lengh: 2250 meters).
The aircraft ran off the end of the paved overrun area and came to a stop
immediately behind it with all wheels on soft ground. There were no
reports about any injured persons. The nosegear collapsed when the 737 ran
into deep mud. The airport was forced to close all operations for one hour
until he 737
was removed.

Inofficial
sources reported the crash of a Antonov An-12 aircraft on landing at
Shindand Air Base und sketchy circumstances. There was no confirmation
about the fate of the crewmwmbers on board. The cargo consisted of 2,4
tons of mail for the US troops in Afghanistan. Among the cargo may have
been a number of packages containing absentee ballots for the US
presidential election on Nov. 6th. Most of the cargo was said to have been
destroyed by post crash fire. There were no further information about the
accident itself and the identity of the aircraft involved. The identity of
the aircraft is still unconfirmed.

On take-off
from runway 05 at Glasgow-Abbotsinch Airport, the pilots rejected the
take-off run because of smoke
was noticed in the cabin. The 737 came to a stop of the runway and an
emergency evvacuation was initiated during which an unconfirmed numer of
injuries occured. Operations at the airport came until the 737 was removed
about 2 hours later.

On landing
runway 07 in rainy conditions, the aircraft
experienced multiple tire bursts causing a loss of directional control.
The aircraft began to skid sideways until it came to rest near the end on
the runway pointing about 70 degrees to the right. It is understood that
no passengers od crewmembers were injured. The airport was forced to shut
down its operations until the damaged tires were replaced and the A321 was
towed off the runway. Unconfirmed reports indicate a possible fault in the
braking system of the aircraft may have contributed to the incident.

On
landing the aircraft veered off the the side of the runway where it came
to rest with all undercarriages on soft ground. The nosegear dug into a
drainage ditch. There were no injuries and
the airport operations were not disturbed. Earlier on approach the pilots
reported a technical failure affecting the wheel brakes and the nosegear
steering mechanism. The gear extension had to be done manually. The aircraft was loaded with a
cargo of electronic consumer products.

Whilst flying
en-route at 22000 ft. from Armenia to Syria, the aircraft was intercepted
by directed by turkish air force planes (~10:53L) to land at Erzurum
Airport. About 10 minutes later the An-12 made a normal landing there.
Turkish media reported the An-12 was suspected to carry weapons or
military equipment for the Syrian Army. After the aircraft was searched,
it continued its flight to Aleppo departing Erzurum about 90 minutes later.

In the
beginning of the push-back maneuvre from the parking position, probably
during the engine start-up sequence, the smell of smoke spread was felt by
passengers and crewmembers. Moments later, dense smoke appeared on
the flight deck and inside the cabin prompting the crewmembers to stop the
push and to carry out an emergency evacuation via slides and overwing
exits. It is understood that at least 27 passengers received medical
treatment in hospital because of smoke inhallation. Two of them suffered
serious injuries when they jumped down the wing after they evacuated via
the overwing exits. Initial reports of the appearance of flames inside the
cockpit are unconfirmed yet.

On landing
runway 15 the
aircraft suffered the collapse of its left maingear resulting in serious
damage to its left wing and engine. Both crewmembers remained unhurt. The
MD-11 came to rest on its left engine #1 and the left wingtip. The left
maingear became bogged underneath the left inner wing, where it sheared
off the inner flaps. Its center gear sustained damage as well where at
least one burst. The incident closed the only runway at
Viracopos Airport for many hours until the cargo was offloaded and the
aircraft was removed. At the time local weather consisted
of strong winds in excess of 25 knots from 130 degrees.
NOTE: the same aircraft was previously involved in a another landing
accident in Montevideo, Uruguay in Oct. 2009 when its right maingear
suffered structural damage.

engine #2 forcing the pilots to discontinue the climb at 3000 ft. The
engine was shut down and the A330 safely returned to Charles de Gaulle

Very soon
after take-off from runway 01L(eft) loud bangs were heared coming from the
right engine #2. The engine developed puffs of smoke and streaks of falems
were seen behind it. The pilots shut the engine down and safely returned
to Gardermoen Airport about 15 minutes later.

The aircraft
was taxiing from the gate at Lagos MMA2 comple to the assigned take-off
runway 18L when one of its wingtips struck a stationary gully emptier
vehicle which was parked close to the taxyway in a drainage ditch.
According to airport offcials and the FAAN airport authority, the Fokker
100 aircraft sustained only minor damage to its outer wing but was unable
to carry on with the flight and had to enter maintenance. The
passengers left Lagos in a different aircraft with a delay of about 2,5
hours.

While the
aircraft was parked empty, a
fire broke out in the nose of the aircraft believed to have started in the
electrical ground supply system while a ground power unit was connected.
Heat and flames caused visible damage until firefighters arrived quickly
extinguished the fire. The next day, the aircraft carried out a low-level
ferry flihgt from Philadelphia to Salisbury for further damage assessment
and possible repair.

On
take-off in heavy rainshowers and thunderstorm activity, the pilot
apprently lost control and crashed behind the end of the runway
where it impacted into the ground nose first. It is understood that two
persons on board did not survive the accident, among them the pilot. The
other occupants were critically injured. The airport was closed for
all traffic until rescue operations were ongoing. The foreward fuselage of
the aircraft was destroyed by impact forces. According to a preliminary
accident investigation by the Eastern Caribbean Civil Aviation Authority,
found substantial amounts of water in the fuel system of the right engine.

at Montserrat in 2011

The aircraft
was flying on behalf of the Sudanese Air Force when about 10 minutes after
take-off, the pilot reported the failure of one, later of two engines. The
pilots turned around for a planned emergency return to Khartoum
International Airport, when they were unable to maintain height. When it
becomes obvious that a landing at their destination was impossible, a
forced landing in the desert about 40 KM southwest of the capital (other
sources say 70 KM) in the vicinity of the proposed new airport for
Khartoum. On landing the aircraft disintegrated and was destroyed.
NOTE: The average age of the An-12 fleet of Azza Transport is 45
years.

During
pushback from Terminal1, the tail of the Air Europa aircraft came into
contact with the other A330 which was stationary at the time, clipping its
upper winglet of the left wing. It is understood that there were no
injuries in the occurence. Damage to the Air Europa aircraft was only
minor, the Orbest A330 needs to remain on the ground for repairs.

While the

received major damage by a catering truck to the underside of a wing when
it was parked at gate C23. At the time, no passengers were on board and no
one was hurt. The truck left a large hole forcing the operator to cancell

Immediately
after departure the left nosegear wheel seperated the its axle and fell to
the ground. The pilots continued the climb to a lower altitude, and
returned to land in the direction of runway 29 where it came to a
standstill without forther problems. The pilot touched down in a part
of the runway which was prepared by dust and sand to reduce possible fire
risks.

The 1934
vintage biplane aircraft - named "Riama" - was on a flight from
the Norra-Aus Fly-In at Monto (about 180 km west of Bundaberg) to
Caboolture when it entered the cloudbase and was flying unintentional in
instrument meteorological conditions (IMC). The pilot - a 68 year old
experienced veteran pilot - requested guidance from air traffic control to
exit the clounds to resume visual flying conditions again. The aircraft
then failed to respond to radio calls and is was declared missing. Despite
a extensive aerial search operation the aircraft remained undetected for
two days. On Oct. 3rd, scattered debris of the Dragon was found in a dense
wooded aerea on a hillside of near Mount Kandanga, about 100 km north of
Brisbane. There were no survivors. The aircraft was totally destroyed by
impact forces. The ATSB began investigations.

On landing
runway 12, all four maingear tires deflated resulting in lots of sparks
and smoke until the aircraft came to a halt on the runway. The runway
remained closed for al ltraffic for about 2 hours until the A319 was
removed by a towing vehicle .
