On departure
ar Minneapolis-Intl Airport, the aircraft lost the left nosegear wheel
which dropped to the ground from a low altitude immediately after getting
airborne. The flight was continued to Duluth wheere the pilots made a
low-pass over the runway to allow airport personnel a visual inspection.
The missing wheel was confirmed. The aircraft entered a holding pattern
east of the airport until it made a safe landing on runway 09. The lost
wheel has not been recovered so far.

On landing
runway 15 in strong crosswinds, the aircraft was apparently caught by a
gust of wind veered off the runway to the right. The aircraft ran over
unpaved surface beneath the runway for some seconds until the pilots
managed to steer it back onto the runway leaving a large red dustcloud
behind. The aircraft then proceeded normally to the parking area. No one
inside the aircraft was injured. Reportedly the aircraft received unknown
damage to its undercarriage.

The aircraft
crashed on the banks of Manohara River, only 2 minutes after take-off from
Kathmandu-Tribhuvan Intl AP. On impact in flat terrain only about 500
meters away from the beginning of runway 02, the aircraft caught fire and
burned. Only the tail of the Do228 remained intact. There were no
survivors. Weather conditions have been reported as clear skies with local
mist and calm winds. Early reports indicate the aircraft encountered
birdstrike during the initial climb phase and a fire may have broken out
in flight. According to preliminary findings by the nepalese authorities,
the a large bird (poss. eagle) collided with the right hand engine at an
altitude of about 50 ft. The impact caused a fire and the seperation of an
engine part which impacted the vertical tail jamming the rudder and making
the aircraft uncontrollable.
NOTE: The accident marks the 2nd fatal hull loss of a Do-228 in Nepal
in 2012 after an Agni Air Do228 crashed near Jomson in May.

The aircraft
was taxiing for departure from runway 02 in darkness, when the right
maingear rolled of paved surface of taxyway "Bravo" where it dug
into soft ground preventing the aircraft to move any further. No one
inside the 737 was hurt. The aircraft was chartered to fly NASCAR
officials and race team members. The aircraft was pulled back onto the
pavement by 0800 in the morning.

While the
parked aircraft was prepared for departure, a baggage cart collided with

There were no reports about any injured persons. According to airport
sources the baggage driver was wearing an pair of open sandals which strap
got caught in the accelerator causing it to speed foreward until it rammed
the nose of the ATR.

On approach to
runway 22, the Airbus A330 experienced a problem with its nosegear which
pointed at an 90 ° angle to the side. After two other approaches without
making any progress, the aircraft entered a holding pattern to the south
off Long Beach. After about 15 minutes, the pilots elected to make an
emergency landing land with
their tilted nosegear on runway 31L, but however when the maingear were
touching down, the
nosegear suddenly went into the correct position and a safe landing was
made.
Authorities are investigationg the incident.

The arriving

pilots spotted the other Pegasus 737 which was under tow about to cross
their runway ahead of the. At an altitude of 350 feet, the approach was
aborted and a go-around initiated. The flight circled the airport and
landed safely about 10 minutes later. The other 737 was towed across the
active runway from the main apron to the maintenance area.

runway 26 in windy conditions, the aircraft experienced windshear on short
final (about 500 ft AGL) resulting in a harder-than-normal touchdown
(+1.75G) followed by a temporary loss of directional control. The 737
departed the right side of the runway where it ran across soft ground for
about 400 meters before the pilots were able to steer back onto the runway.
The landing run was safely completed and no one inside the aircraft was
hurt. Damage occured to the nose and maingear tires. At the time of the
incident, local weather consisted of thunderstorm activity, intense rain
and reduced visibiulity to 1100 meters. According to a Ministry of
Transport investigation, the loss of directional control was also
attributed to hydroplaning because the runway was wet from recent rain.

On landing
runway 33 in heavy downpour, the aircraft skidded off the end of the
runway. The aircraft came to rest with all gear on the grass on
downsloaping terrain when it was turning to the right to vacate at the
last exit "A1". All passengers and crewmembers remained unhurt.
Birmingham Airport has one single runway 15/33 of 2600 meters in lengh.
Damage to the aircraft is believed to be minor. The airport was forced to
shut down all operations for larger jets for more than 3 hours until the
737 was removed.

The aircraft
received substantial damage during an aborted take-off from Dolpa´s
gravel runway after its left wing became entangled with a fence. The
pilots evetually lost directional control and ran off the end of the
runway by some 50 meters where it hit some rocks and stones before coming
to a stop. it is understood that nobody on board was harmed. Dolpa has a
gravel runway of about 500 meters on lengh.

During the

The A320 was
in its climb after take-off from Damascus Airport when it was hit at around 12000
ft. by the rotor of a
Mil-Mi Helicopter operated by the Syrian Armed Forces. The helicopter was
previously engaged in combat fighting with the "Syrian Liberation
Army". When it lost altitude it came into the path of the climbing
A320. One of its rotor blades sliced through the vertical fin of the A320 cutting
it nearly in half. While the helicopter crashed near Douma and both pilots
were killed, the
Airbus was able to land safely at Damascus Airport.

The
aircraft was prforming an ILS-CAT II approach to runway 09 at Algiers in
low visibility conditions with ground fog limiting foreward visibility to
around 100 meters. On final approach the aircraft descended below the
glidepath and drifted to the right of track until the 737 made a touchdown
at the very beginning of the paved ground, well ahead of the markings,
showing the beginning of the runway. It took out a number of runway edge
lights resulting in minor damage it its tires before the pilots made a
safe landing rollout and proceeded normally to the parking stand.
NOTE: official documentation of runway 09 approach showed a minimum
distance of 550 meters runway visibility is required using the CAT II
autoland approach. At the time, flight AH 6231 was the only one that did
not divert to alternate airports, all other traffic elected not to land at
Algiers.

altitude and returned to Honolulu, Hawaii where it made a safe landing
about 4,5 hours after departure and was able to steer off the runway. On

The A321 was
taxiing to its final parking position in front of the terminal,
when its left wingtip hit the wing of the 737 which was stationary at the
time at the neighbouring gate resulting in damage to both aircraft.
Both departure flights to Turkey were cancelled. It is understood that no
person inside any aircraft was hurt in the event. Preliminary
investigation showed the A321 pilots were instructed to proceed to gate 05
but erroneously the aircraft followed the ground markings to position 05B
which is much closer to the adjacent position 04, where the 737 was
standing.

During landing
on runway 10, the aircraft veered to the side of the runway where it
knocked down two runway edge lights with its right maingear until the
pilots managed to steer the A330 back to the middle of the runway. The
landing run was safely completed and the aircraft proceeded normally to
its parking stand. The aircraft received only minor damage. At the time of
the landing local weather consisted of continous rainfall. The runway
remained closed for about 50 minutes.

The aircraft
crashed on approach to Palana Airport in Russia´s Pacific Region after
radio contact was lost. The accident site is at an elevation of 500 ft.,
about 12 km
south of Palana Airport in an area of rising terrain. The An-28
hit wooded hillside and broke apart on impact. It is understood that four
passengers who were sitting in the back of the cabin have survived the
crash. At the time of the
accident, local weather consisted of low ceiling, reduced visibility and
intense rain. Initial finding of the wreckage showed the aircraft lacked a
terrain warning system (GPWS). Its installation was required by russian
legislation as of Oct. 1st as the latest. Authorities began to investigate.

The aircraft
was about 80 miles southeast of its destination, when it suffered a
complete electrical failure. All four power generators stopped working
leaving the pilots on battery power only. The pilots were able to restore
power from two generators and continued to proceed to Anchorage. With its
anti-skid system without power, the pilots were forced to use manual
braking on landing runway 07L(eft) in darkness, causing 14 (of 18) tires
to blew out. The 747 came to a stop on the runway but was unable to vacate.
Runway 07L/25R remained closed for over 12hours until all tires were
replaced and the aircraft was towed away.
NOTE: Anchorage´s parallel runway 07R/25L was closed days before
because of routine overhaul works.

The pilots
aborted the take-off run after they noticed abnormal indications from one
of the left engines (GEnx-2B67). The take-off was safely aborted and the
747 taxied back to the apron. Post incident runway inspection showed
numerous engine fragments on the departure runway indicating that an
uncontained engine failure may have occured. It is unclear weather there
was any additional damage to the aircraft apart from the engine. The
runway remained closed for cleaning purposes for the next 3 hours. Inspection
however showed the engine failure was contained and all debris is believed
to have exited the engine via the exhaust pipe.

At the
beginning of the final approach to runway 31L at Boeing Field, the
aircraft suffered seperation of one of the main gear doors. The door is
about 1,5 x 1 m in size and fell to the ground at East Hill, Kent, a
Seattle neighbourhood, wherre it came to rest in a street fortunately
without hitting anybody on the ground. No property was damaged. The 767
continued the approach and landed normally at Boeing Field. Authorities
are investigating.

Immediately
after touchdown on runway 24 at Cologne-Bonn Airport smoke was observed
coming through the air-condition into the the cabin for only a couple od
seconds. At
least eleven injuries have been reported by Airport Officials. Passengers
deplaned normally via mobile airstairs on the main apron. Earlier, the
same 737 was forced to return to Hannover on a flight to Gaziantep because
of strange fumes in the cabin. After repairs the aircraft was dispatched
for the flight to Cologne. Preliminary investigations showed that a valve
in the engine oil system has malfunctioned.
NOTE: This
particular aircraft (D-AXLF) was previously involved in a landing accident
at Belgrade in Oct. 2008.
NOTE2:
Another 737-800 of XL Airways (D-AXLG) was substantially damaged in a ground
incident at Frankfurt in early August 2012.
