aircraft was on approach to the small airfield of Namoya in Maniema
Province, when contact was lost. the pilot of another plane reported
seeing the wreckage of the An-28 in wooded area with no apparent
indication of survivors. Recue teams found one passenger and one
crewmember alive. The other three on board were found dead.
The aircraft was on a mixed passenger-cargo flight from Bukavu.

The
737 was taxiing to its gate near Concource "C" after landing
when its left wing knocks down a light pole causing unspecified damage to
the left wing leading edge. The pilots elected to stop the aircraft on the
tarmac and all passengers were deplaned via mobile airstairs and bussed to
the terminal. There were no reports about any injured persons. The
incident is under investigation.

the early stages of the take-off run the pilots noticed a double nosegear
tire burst prompting them to cancel take-off immediately. The aircraft
came to a safe stop on the runway and was later towed to the apron. At the
time of the incident, local weather consisted of dense fog with foreward
visibility limited to about 200 meters only. Initial inspection showed
damage to a number of runway lights possibly broken when the A330 rolled

to indian news reports, the piolots were forced to interrupt their
take-off at Jammu, when they saw a military bus on the runway ahead of
them. The take-off was cancelled and the A320 safely slowed down. The
reason why the bus did not hold before the runway is under investigation.
The bus was heading to a military apron loaded with passengers who were
stranded after the only road to Srinagar was blocked by heavy snowfall in
the region. The Indian Air Force sent IL-76 transport planes to fly

Immediately
after getting airborne at Naha, whilst crossing an altitude of about 1000
ft., the aircraft collided with a flock of large sea gulls or albatrosses.
One bird smashed through the nose and became buried inside the nosecone
resulting in a large hole. Despite the damage the pilots continued their
survey flight and proceeded to their planned destination Ishigaki, where a
dafe landing was made about an hour later. Japanese authorities are
investigating.

after take-off the pilots reported an onbourd fire in the passenger cabin,
stopped further climb and initiated an immediate landing return without
delay. However on final approach, the pilots reported the fire was
extinguished prompting them to discontinue the approach and performed the
standard fuel dump procedure before sufficient weight was lost for landing.
The 777 came in for a 2nd landing attempt and proceded normally ot the
gate. Early information showed that the fire began in the vicinity of a
passenger seat in business class. Other travellers reoirted seeing small
flames on the armrest before cabin crewmembers discharged a fire bottle
and the flames went out. The airline indicated that apparently a cigarette
lighter was the cause. The owner of the cigaratte lighter not identified
yet. |

After

turn, the nosegear began to slide coming to rest in soft snow. The
aircraft became immobile and the runway was closed until the MD-88 was

During
the approach to Guayaramerin, the pilots noticed that the landing gear
would not extent. The crew were forced to make a belly belly landing on
runway 34 and came to a standstill after about 1300 meters. (runway lengh
1764 meters). The aircraft came to rest on its fuselage and left wing,
resulting in substantial damage. There were no reports about any injuries.
Due to the blockage of the only runway, Guayaramerin Airport was forced to
close for a longer time.
NOTE: This
particular aircraft was involved in a nearly similar accident at
Rurrenabaque in March 2011.

| On
landing runway 06 the aircraft suffered a double tire burst on its left
maingear resulting in a drift to the left. The pilots were able to stop
the aircraft at the edge of the runway but still on paved surface. No one
on board was injured. Belem Airport was forced to close for 6 hours until
the aircraft was removed. |

When
the aircraft was doing a U-Turn at the end of the runway after landing,
its left maingear ran off the side of the paved surface. A short distance
later it collapsed or dug in resulting in unknown damage. The left outer
wing came into ground contact. There were no reports about any injuries on
board. Sampit Airport was forced to close for all traffic for at least 24
hours because of unsufficient removal equipment..

Excessive forced was needed to flare the aircraft properly before
touchdown. The aircraft proceeded normally to its gate position and was

aircraft was descending towards Brussels for a landing on runway 25L, when
it experienced an area of intense icing. The pilots then reported an
engine shutdown because of low oil pressure indications. The aircraft made
a safe landing at Brussels on the remaining engine.

| The
A380 was instructed to taxy to the holding point to runway 16R(ight) using
taxyway "Golf" and "Alpha", however it missed the left
turn into "Alpha" and continued to roll straight foreward until
it entered the active runway 16R where at that time the JAL aircraft was
accelerating for take-off. Quickly the JAL-pilots rejected the take-off at
slow speed and came to a stop before the A380. Both aircraft later
departed to their resp. destinations without further problems.

taxy for take-off in darkness and limited visibility, ATC cleared the
aircraft to cross runway 35R and later issued a take-off clearance from
parallel runway 35L, however the pilots lined-up on runway 35R instead and
took off, fortunately without coming into conflict with other ground
traffic. The aircraft preceeded normally to its destination. Moroccan
authorities are investigating. |

| During
take-off rotation from runway 24 at Schiphol Airport, the 767 is suspected
to have suffered a minor tailstrike. Ground observers reported seeing a
small puff of smoke behind the tail. The pilots were informed and the
flight was cancelled. After 2 hours of holding, the 767 returned for a
safe landing on runway 27. Damage is believed to be minor (poss. only
tailskid assembly) and the flight departed again about 2 hours later. At
the time of the incident, stormy conditions prevailed at the airport. |

After
a normal landing on runway 32, the aircraft began to slide when it was
about to vacate into a taxyway causing it to depart paved surface. The
nosegear and one of the maingears apparently ended in soft snow. There
were no reports aqbout any injuries. Weather conditions at the airport
were below freezing level at the time.

| On
take-off run at Vancouver a tire received damage and loose fragments were
found lying on the runway. Because of no abnormal indications, the crew
elected to continue the transpacific flight. However, in the 2nd half of
the flight the pilots received a cautionary message for the center
hydraulic system. The aircraft laded safely but made a full stop on the
runway and needed to be towed to the terminal. Official investigation
revealed damage to the #4 right main gear tyre. Hydraulic lines and wheel
brakes line was also found damaged. |
