The
aircraft departed Medan at 07:18L and proceeded on a westerly heading
until the pilots failed to respond to ATC calls and the flight declared
missing. Later an emergency locator signal (ELT) was received and the
aircraft was found to have crashed in densely wooded terrain near Bahorok,
about 70 kilometres north west of Medan. Rescue teams found no survivors. All on
board died in their seats. The aircraft impacted ground nose first leaving
its foreward section totally cruashed while the aft part of
the aircraft and the wings appeared to be intact.

fully compliant with indonesian safety regulations).

DC-9 made an extremely hard touchdown causing both engines to break free
of their support fittings. With both engines hanging down in a ~80-degree
angle, the pilots slowed down on the runway to a safe stop and both
engines were shut down. There were no reports about any injured persons on

| During
climb-out after departure from JFK Airport, a leaking oil seal in engine
#1 (PW 4000) caused nauseatic and toxic fumes in the cockpit and passenger
cabin. A lot of passengers complaint about headache and feeling unwell.
All crewmembers were also affected by the fumes, most seriously, the
purser. The pilots elected to continue the flight as the odour reduces
during cruiseflight but increased its intensity during descent again.
However, the aircraft landed safely at Berlin. |

The
aircraft was near the end of a sightseeing flight approaching runway 02 at
Tribhuvan Airport when it flew into an area of low clouds, fog and
precipitation until it struck the ground about 8 km south of the runway
threshold. The Beechcraft broke apart and exploded. There were no
survivors. The minimum safety altitude was 6500 ft, ground elevation at
the crash site was about 5200 ft. The flight was carrying tourists from
Kathmandu around the Mount Everest and back, offered by Buddha Air as
"The Everest Experience". At the time of the crash, local
weather consisted of overcast skies with low ceiling, fog and rain.

land on Great Slave Lake in Yellowknife Bay when it apparently clipped
some power lines and crashed into a residential neighborhood. Both pilots
received fatal injuries and a number of passengers were injured, some of

minor damage. The pilots were able to decelerate safely and vacated into a

further approach clearance, when a loss of seperation occured bringing
them to a distance of about 1 mile in between when flying at the same
altitude. Reportedly, initial ATC instructions to maintain vertical
seperation showed no response. The conflict was resolved when both crews
followed their resp. TCAS resolution advisories. Both flight landed
normally later at Hong Kong. The airport was forced to close temporarily
because of adverse weather conditions. |

Following
a long landing in twilight and rainy conditions, the aircraft overran the
end of runway 35 at Mariscal Sucre International Airport coming to a stop
on a concrete wall. A number of passengers are known to have received
various injuries. The aircraft lost its entire undercarriage and the nose
section received substantial damage. Runway lengh at Quito is 3125 meters.
Runway 35 has a displaced threshold.

the pilots made a safe landing at Southland Airport, about 8 miles west of
their intended destination Lake Charles. At the time of the landing it was
dark but the skies were clear and visibility was reported being "excellent".
All passengers were later bussed to Lake Charles. Both airports are prone
to be mixed up because of their similar runway layout. It is understood
that his was the 3rd case within the last 15 years after two such landing

broke apart at the Volga River banks, about 1,4 kilometers west of the
runway. A fire broke out. Parts of the
wreckage came to rest in the water. It is understood that the vast majority of
the 45 people on board did not survive the crash. At the time the Yak-42 was
operating on a charter flight carrying the entire team of the Lokomotiv
Yaroslavl Icehokey Club for the season opening match agasinst Dynamo
Minsk.
Russian aviation regulatory authority Rostransnadzor suspended all 57
Yak-42s in Russia pending the outcome of inspections of the fuel-system
and engines.
NOTE: This accident marks the 8th hull loss event for a Yak-42 aircraft
and the 7th hull loss of a commercial airliner in russia in the current
year.

communications were lost. After a massive search and rescue operation the
aircraft was found to have crashed two days later. The wreckage was
located in a lagoon. Only one male passenger is known to have survived the

| Whilst
flying en-route at 41000 ft., the aircraft experienced a sudden violent
roll maneuvre to the left. The aircraft rolled to an almost inverted
position to a maximum angle of 131,7 degrees and the nose dropped down to
35 degrees before the roll ended and the 737 established level flight
again at 34700 ft. Investigation showed the copilot was alone in the
cockpit at the time. In an attempt to unlock the cockpit door to allow the
captain to enter the flightdeck again he inadvertently operated the rudder
trim switch. Two flight attendants were injured. The incident is under
investigation. |

landing in rainy conditions, the aircraft ran off the end of runway 7 coming to rest with its undercarriage
damaged on the grass. There were no
reports about any injures persons on board and all passengers left the
aircraft unhurt. The aircraft suffered a fuel
leak resulting in a massive spillage.

After
an apparent normal landing on runway 32, the nosegear ran into the hook
net barrier whilst doing a 180-degree U-turn at the end of runway
resulting in a tire burst and a subsequent complete failure of the
nosegear. An emergency evacuation was initiated in which 11 passengers were
hurt. Damage to the aircraft is being considered as repairable although
the aircraft sustained heavy damage to its foreward underbelly aft of the
nosegear bay doors. At the time Mashhad Airport was in a
noise-abatement configuration with reciprocal useage of runway 32 for
landings and 13 for take-offs.

After
landing on runway 27 in rainy conditions, when the A340 was about to turn
right into the high-speed taxyway "N8" (second last exit) it ran
off paved ground and all gear became stuck in mud. All passengers left the
aircraft via mobile airstairs. Due to continous downpour, the airport was forced to close runway
09/27 for more than 75(!) hours until the A340 could be removed in the
morning of Sep. 05th. Damage to the
aircraft is considered to be minor.

approach a warning light illuminated indicating an unsecured
left maingear. After fruiless attempts, the pilots were unable to lower
it resulting in a landing with the left gear in the retracted position.
During deceleration process on the runway, the left wing made ground
contact before the aircraft came to a stop. There were no reports about
