On
approach for runway 18 at Avalon Regional Airport, about 30 km southwest
of Melbourne, the aircraft descended below the prescribed minimum safe
altitude of 3700 feet when it was about 15 km north of the airport.
It was measured the flight was over 500 feet below the minimum. The pilots
then realised their error and climbed again to a higher altitude before a
safe landing was made. An investigation was opened.
NOTE: It was the 2nd time that an Tiger Airways flight violated minimum
altitudes after a comparable incident on June 7th occured near Melbourne. On
July 2nd, Australian Civil Aviation Authority (CASA) ordered Tiger
Airways to remain on the ground after further investigation reportedly
identified a number of safey deficiencies in terms of crew training and
maintenance procedures. |

On
(NDB) approach to runway 02 at Petrozavodzk in near darkness and thick fog the aircraft deviated
from the approach path until it collided with trees. The Tu-134 went down on a motorway
breaking apart and bursting into
flames on impact. Only 5 people survived the crash
with various injuries. The wreckage came to rest in a inverted position about
500 meters south of the airport boundary. It is understood runway 02 at Petrozavodzk is not equipped with
an automated instrument landing system (ILS). At the time of the
accident, local airport weather consisted of fog with visibility limited
to 2,1 km, 400 meters ceiling and calm winds. There are unconfirmed report
the aircraft may have severed powerlines before ground impact resulting in
a momentary power outage of the runway lightning and navigational aids.
Russian President Medvedev ordered the Minstry of Transport to remove the
Tupolev Tu-134 from service at the end of the year.
NOTE: This accidents marks the 66th hull loss of a Tupolev Tu-134 and
the first hull loss for RusAir.

The
cargo aircraft ditched into water about 3 kilometers to the south of its
destination when it was on final approach to runway 34 at Libreville International Airport. All on board survived the accident and were rescued.
The plane impacted water a few hundred meters from shore, close to the
former "Hotel Dialogue". According to
ground witness reports both propellers were not turning before the
aircraft went down. The aircraft remains intact after it hit the water. Parts of the tail and wings were still afloat when
rescue boats arrived.
On June 8th the gabonese government banned all flights with Antonov An-12,
An-24 and An-26 aircraft in the country.

| On
final approach to runway 16, the 737 initiated a go-around maneuvre. Due
to the fact that a thunderstorm cell was directly in the missed-approach
flightpath, the pilots were cleared to turn right to 300 degrees and to
climb to 5000 ft. again. However, the 737 rolled out on a heading of about
340 degrees (opposite landing direction) and came into seperation conflict
with the approaching korean 747. Both aircraft passed each other in
reciprocal directions with 6,5 miles horizontally and with seperation down
to zero. |
