After
an aborted take-off the pilots were unable to stop the aircraft within the
lengh of the runway, resulting in an overran. The Yak-42 came to a halt
about 50 metres further on soft grass. There were no injuries reported
among passengers or crewmembers. The airport was forced to close for about
3,5 hours. A failure in an engine was given as a reason for the abort.
Damage to the aircraft is likely to be repairable.

| The
Airbus A320 erroneously rolled onto an active landing runway in darkness
whilst taxiing for take-off from runway 27. While travelling on the
inactive runway 14, the pilots missed the turn onto the parallel taxyway
"N1" only to find themselved at the runway intersection. At this
time the 737 was on final approach to land on runway 27 and was forced to
make a go-around meneuvre by ATC. The A320 quickly turned onto the correct
taxyway while the 737 returned for a safe landing. According to a
statement by the Indigo-crew the edge lights of the taxyway were not
working at the time. |

aircraft were involved in a near mid-air collision to the south of the
airfield. The A319 aborted its landing when it was on short final to
runway 14 due to unfavourable tailwinds and overflew the airport in low
altitude. At the same time the 747 was doing ts take-off run on runway
25R(ight) and got airborne when the pilot inside the A319 got instruction
to turn right to a heading of 300 degrees when both aircraft came into
conflict with each other. Their closest distance was measured with 100
feet vertical and 0,3 miles lateral before the conflict was resolved.
Weather at the time was described as "good" with more than 10
statue miles visibility. The incident is under investigation. |

On
final approach to runway 33 in poor visibility conditions, the aircraft
descended below the propper approach path and impacted terrain about 500
metres short of the runway. The Bandeirante came down in a soy bean field
where it came to rest on solid soil after about 150 metres with all its
undercarriage collapsed. Substantial damage occured to both wings end
engines. The right wing sustained additional damage after it struck an
obstacle. Both pilots sustained minor injuries. At the time the aircraft
was operating a cargo flight without passengers. Only NDB/DME approaches
are available at Cascavel Airport.

The
aircraft crashed en-route in northern Afghanistan after radio contact
was lost about 15 minutes before landing at Kabul. The scattered remains
of the aircraft were found 2 days later south of Salang valley. Search and Rescue
are hampered by rugged terrain the limited accessability to the wreckage
site and unfavourable weather conditions. There were no survivors. Local
weather at the time was described as "bad" with lots of clouds,
snowfall and flooding in lower parts of the region.

| In
the early stages of the cruiseflight, the pilots suddenly reported smoke
and a small fire in the cockpit. The pilots elected to divert to
Washington Dulles. The fire was brought quickly under control by on-board
devices. Post flight inspection revelaed the source of the fire was the
windshield heating system. Fire from the windshield heaters had caused a
number of emergency diversions on previous occations. |

Only
minutes after take-off from remote Godo Olo airstrip, the aircraft entered
an area of bad weather and radio contact was subsequently lost. The
wreckage was later spotted by the crew of another Blue Wing aircraft about
5 km north of the airfield amidst dense jungle. There were
no survivors. The national Aviation Authority CAAS immediately grounded
the remaining two AnN-28 of the airline until further notice.

As
part of a scientific survey mission the sky-equipped aircraft landed on
the ice cap when its right main gear began to sink. The pilots engaged the
emergency locator transmitter to alert rescue services. All persons on
board were evacuated by a helicopter 2 hours later. At that time, the
aircraft was about 50% submerged in ice and water. It is believed the
aircraft was given up and is damaged beyond repair.
NOTE: This is the 2nd major incident of a Kenn Borek aircraft within a
month after another DHC-6 was involved in a take-off overrun at Melville
Islands on April 14th. |

Very
shortly after take-off at Newark Liberty International Airport, the pilots
received indication about a hydraulic leak. Simultaneously, ground
witnesses observed fluid leaking out of the left main gear area. The
flight was cancelled, and the Boeing 777 safely landed at Newark on runway
22L. People on the ground saw the aircraft dumping fuel to reduced its
landing weight. After landing, the pilots requesteed a ground asisstance
for further taxy.

which was observed by a crew from another aircraft. The pilots were
informed and elected to cancel further flight. After enough fuel was
jettisoned the 747 returned for a safe landing on runway 34L about 80

After
an uneventful landing on runway 36L, ground crews noticed substantial
damage to the underbelly close to the main undercarriage. A hole and tire
marks were seen in an unpressurized middle segement of the airframe. It is

aircraft veered off to the left where it came to rest on the grass. None

On
landing runway 02 at the Fabio Alberto Leon Bentley Airport at Mitu, the
aircraft ran beyond the end of the runway. It travelled through the
airport fence and came to a halt in the grass on a nearby field. It is
understood there were no injuries among passengers and crewmembers. The
hull appears to be relatively intact but the undercarriage collapsed when
the aircraft rolled over soft ground resulting in substantial damage. At
the time of the accident, the runway was wet from recent rain. The runway
02/20 has an asphalt surface and is 1.795 metres in lengh.
NOTE: SATENA also operates on behalf of the Colombian Air Force (FAC).
In the last 3 decades it lost 9 aircraft in accidents where 84 people died.

approach to runway 16 the aircraft dropped below the propper approach path
until the propeller of the right engine struck a high tension cable about 1 km away
from the runway. The pilots managed to maintain full control of the
aircraft and made a safe landing. No one on board was injured. Damage to
the aircraft was limited to the right propeller blades. The cable was 9 metres
above the ground. At the time of the incident, local visibility was
described as 'excellent'. Elba Airport has a special approach procedure for
inbound aircraft from the north. Due to surrounding mountain ridge no
incoming plane can land in direct line to runway 15 but has to make a
50-degree-turn before crossing the airport boundary. The incident is under
investigation.
