In
the middle of the cruiseflight at 38.000 feet (FL 380) the A330
encountered a zone of thunderstorms associated with severe turbulence,
prompting the pilots to radio a "mayday"-call. That call was
relayed to ground stations by a TAM flight. The pilots then
descended to a lower altitude (without ATC clearance) and normal flight condition were achieved
again until the remainder of the flight. The A330 landed safely at Paris.
NOTE: The flight was flying roughly the same area where Air France
flight 447 crashed into the sea on June 1st 2009. Turbulent flight
conditions prevailed on this flight as well. |

The
pilots made a forced landing into trees after the aircraft ran into some
kind of unspecified technical troubles. It is understood, that all persons
on board have survived the accident. One wing was broken, but fortunately
no fire broke out. Local media reported the aircraft came from a
neighbouring country and was on a non-commercial flight on behalf of the
US Government. It also was reported, that military personnel was among the
passengers, flying to Mali to assist local police in anti-terror missions.

aircraft were flying en-route in opposite directions, the 747 at 34.000
feet (FL 340) was heading west, the Gulf Air aircraft at 35.000 (FL350)
was heading east when suddenly the 747 left its flight level and began to
gain altitude. This triggered a TCAS alert in the other aircraft making
the pilots aware of a potential collision risk. The crew of the Saudi Arabian
aircraft quickly arrested the cimb at 34.200 feet and descended again to
its original altitude. Both flights continued safely to their destinations.
The Saudi crew later explained their vertical seperation violation to ATC
that passenger movement in the aft cabin may have lead to a nose-high
attitude to which the autopilot was unable to respond. |

minute after take-off from runway 19R at Wichita, a loud bang was heared
and the left engine caught fire. The pilots used on board fire bottles to
extinguish the fire and levelled off at 4.000 feet announcing an
immediate landing return. However, on final approach tower personnel
observed flames coming out of the left engine gain. When the MD-87 came to
a standstill on the runway the crew ordered an emergency evacuation in
which one passenger was slightly injured. Airport fire brigade quickly
doused the flames with foam. The extent of damage to the engine is unknown. |

minute after take-off from Isfahan, the pilots experienced problems raising
the undercarriage and elected to return to land. On landing runway
26R(ight) (lengh: 4397 metres) the left main gear caught fire and gave way
before the Fokker 100 came to a stop. The left wing dragged on the ground
resulting in unknown damage. The airport remained closed for 8 hours until
the aircraft was removed. The Fokker 100 was later ferried to Teheran for
final repairs. |

On
take-off run from runway 08 at Port Elizabeth (lengh = 1.980 m) the
pilots apparently lost visual reference to the runway due to intense
rainfall resulting in an runwayexcusion to the eft. The take-off was
aborted when the J-41 ran over grassy ground beside the runway. The
aircraft came to rest about 40 metres beside the runway without serious
dmaage. Later the pilots were able to taxy back onto the runway under own
power. There were no reports about any injuries.

On
landing runway 27(A), the aircraft ran beyond the end of the runway coming
to a stop after travelling over soft ground for about 150 metres. All passengers and crewmembers on board were
able to evacuate without serious injuries.At the time of the accident
visibility was reduced to about 2.5 kilometres and light rain was falling.
Mumbai´s main runway 09/27 is reduced by half to about 1.700 metres on
every tuesday from 12:00 until 18:00L. Only lighter planes up to the size
of an ATR are allowed to operate during that time. This unique "Tuesday
Restriction" is due to continue for another 20 weeks.

visual range: 350 metres), the left main gear and nosegear departed the
left side of the runway. The pilots managed to steer the aircraft back
onto the runway after travelling over unpaved ground for about 370 metres.
The landing run was safely completed and no injuries have been reported.
Minor damage to some tires occured when the aircraft ran over a number of
runway edge lights. At the time of the occurence, local ILS was available
in CAT I mode allowing landings down to a minimum of 550 metres. The

the pilots reported engine troubles about 13 minutes after take-off. The cargo
aircraft contacted the ground short of the runway (probably runway 32)
where it smashed through the airport fence and ran into a ditch, breaking
apart before coming to rest on the grass a few metres away from the runway.

On
take-off the aircraft apparently suffered a failure of its left engine a
few moments after leaving the ground. The pilots made a forced landing on
rough terrain outside the airport where the amphibious aircraft came to a
stop close to a public road to the west of the airport. All occupants
escaped without serious injuries. Aircraft is believed to be a hull loss.

few minutes after take-off from Kennedy intl Airport, the aircraft lost a
engine tail cone which fell into a residential area at Roosevelt northest
of the airport. Fortunately, the falling part did not cause any injuries
or property damage on the ground. The aircraft continued to Tokyo where it
made a safe landing. The missing cone was discovered during a routine post
flight inspection. |

According
to a report of the United Nations Office on Drugs and Crime (UNODC) the
aircraft landed on a makeshift runway in the desert north of Gao. Local police
and Interpol said there are indications that tons of illegal substances (cocaine)
were offloaded and shipped away by road or smaller planes. Upon take-off,
the aircraft may have suffered some kind of power loss forcing the pilots
to make a landing in the nearby Sahara desert. On landing the aircraft was
severely damaged, caught fire and burned to destruction. Presumingly all
on board did survive the crash because no bodies were found inside the
wreckage. According to different sourced the 727 was burnt after landing
and there was never an intention for taking-off again. Malian and International police is investigating the origin of the flight
and ownership of the aircraft. It is now widely belived the 727 is a 1978
built model recently registered in Guinea Bissao (J5-) last known operator
was Senegal based Africa Air Assistance. It was ferried to Brazil in
Mid-2009 and last seen at Fortaleza in August.

#1. The large part fell onto the runway 18/36. Twisted piece of the
cowling remained attached and was bended upwards before the wing leading

| On
final approach to runway 27(A) at Mumbai International Airport the pilots
aimed to a touchdown point short of the runway threshold coming close to
ongoing construction workers who were forced to run out of the path of the
incoming A320. The pilots abandoned the approach and initiated a go-around
maneuvre. However, on the 2nd landing attempt the aircraft again aimed too
short of the runway and made another go-around before a safe landing was
finally achived in a 3rd landing attempt. At the time, runway 27 was
shortened by 1.260 metres due to construction works. The shortened runway
was properly marked and designated as runway 27(A). The incident is under
investigation by Indian DGAC. |

Very
soon after take-off from Mirny Airport, the aircraft entered a steep bank
angle to the right and crashed onto snow covered field. It hit a slag heap
of an abandoned mine about 2 kilometers from the runway where it disintegrated on
impact and burst into flames.
There were no survivors among crew and passengers. The aircraft was
on a ferry flight repositioning empty to Irkutsk. At the time of the crash,
local weather consisted of clear skies calm winds and temperatures
of minus 24 degrees celsius. Russian Ministry of interior
grounded all other Il-76s in the fleet with immediate effect to allow
technical inspections.
NOTE: This accident represents the 4th hull loss of an IL-76 in 2009
(two of them were operated by Russian Ministry of Interior).
