Very shorty after take-off from runway 28, at an altitude of about 500 - 1000 ft., one of the engines ingested a bird causing a loud bang. The pilots cancelled the flight and returned for a safe landing at Galeao Intl Airport.

In the middle of the cruiseflight at 38.000 feet (FL 380) the A330 encountered a zone of thunderstorms associated with severe turbulence, prompting the pilots to radio a "mayday"-call. That call was relayed to ground stations by a TAM flight. The pilots then descended to a lower altitude (without ATC clearance) and normal flight condition were achieved again until the remainder of the flight. The A330 landed safely at Paris. 
NOTE: The flight was flying roughly the same area where Air France flight 447 crashed into the sea on June 1st 2009. Turbulent flight conditions prevailed on this flight as well.

The 737 was pushed back from its gate position when one of its wingtips was struck by the wingtip of the DC-10. Damage to both aircraft is being considered as minor. The visibility was good (10 statue miles) art the time.

The aircraft was flying en-route at 35.000 feet (FL 350) when it was hit by lightning. The pilots cancelled the flight and returned to Gatwick Airport without further problems.

The pilots safely returned to land at Linz following ther detection of smoke on the flightdeck almost immediately after becoming airborne. The smoke also appeared in the foreward cabin.

The A320 was on final approach to runway 35L at Malpensa Airport, when both the pilots and the tower controller spotted a service car was on the landing runway. The pilots initiated an immediate go-around maneuvre and landed safely a few minutes later. The inicdent is under investigation.

NOTE: The accident aircraft was only one week in service with Avient, it was handed over to the airline on November 20th. The accident marks the 7th hull loss event for an MD-11 and the 2nd hull loss in 2009.

In the late stages of the take-off run, the aircraft suffered a tire burst. The pilots continued the lfight and made a safe landing at Berlin.

On approach to Toronto, the pilots reported minor control problems due to ice acculumation of control surfaces and declared an emergency. The pilots were able to resolve their problem, the approach was continued and the Embraer Jet made a safe landing on runway 24R at Leister B. Pearson Intl Airport.

The A380 safely returned to land at JFK International Airport, after the pilots noticed an autopilot malfunction after being airborne for about 90 minutes.

About one hour after take-off from Zurich, the pilots received indication about abnormal indication from engine #3 (inner right) whilst flying in Austrian airspace. The flight returned safely to land.
NOTE: The same aircraft was involved an another mishap the followingday (Nov. 27th) when the pilots could not raise the flaps after take-off prompting another return to land.

On final approach to runway 06 at Moncton Airport, the 727 hit a bird resulting in damage to the horizontal stabilizer in the tail. The pilots continued the landing run without further problems.

The aircraft sustained unknown damage when it suffered a tyre burst during taxy out to the departure runway. The aircraft veered sideways but was brought to a safe halt by the pilots. No injuries have been reported. Among the passengers was sudanese vice president Salva Kiir.

In the early stages of the tke-off run from runway 04L, the left engine suddenly failed and ther take-off was safely aborted. The 737 was later towed off the runway. initial inspection revealed substantial damage to the left engine #1. It is believed the source of the damage was a loose runway light which was jet-blown by a Continental 777 which departed immediately ahead of the 737.

The pilots made a forced landing into trees after the aircraft ran into some kind of unspecified technical troubles. It is understood, that all persons on board have survived the accident. One wing was broken, but fortunately no fire broke out. Local media reported the aircraft came from a neighbouring country and was on a non-commercial flight on behalf of the US Government. It also was reported, that military personnel was among the passengers, flying to Mali to assist local police in anti-terror missions.

Both aircraft were flying en-route in opposite directions, the 747 at 34.000 feet (FL 340) was heading west, the Gulf Air aircraft at 35.000 (FL350) was heading east when suddenly the 747 left its flight level and began to gain altitude. This triggered a TCAS alert in the other aircraft making the pilots aware of a potential collision risk. The crew of the Saudi Arabian aircraft quickly arrested the cimb at 34.200 feet and descended again to its original altitude. Both flights continued safely to their destinations. The Saudi crew later explained their vertical seperation violation to ATC that passenger movement in the aft cabin may have lead to a nose-high attitude to which the autopilot was unable to respond.

A minute after take-off from runway 19R at Wichita, a loud bang was heared and the left engine caught fire. The pilots used on board fire bottles to extinguish the fire and levelled off at 4.000 feet announcing an  immediate landing return. However, on final approach tower personnel observed flames coming out of the left engine gain. When the MD-87 came to a standstill on the runway the crew ordered an emergency evacuation in which one passenger was slightly injured. Airport fire brigade quickly doused the flames with foam. The extent of damage to the engine is unknown.

A minute after take-off from Isfahan, the pilots experienced problems raising the undercarriage and elected to return to land. On landing runway 26R(ight) (lengh: 4397 metres) the left main gear caught fire and gave way before the Fokker 100 came to a stop. The left wing dragged on the ground resulting in unknown damage. The airport remained closed for 8 hours until the aircraft was removed. The Fokker 100 was later ferried to Teheran for final repairs.

On take-off run from runway 08 at Port Elizabeth (lengh = 1.980 m) the pilots apparently lost visual reference to the runway due to intense rainfall resulting in an runwayexcusion to the eft. The take-off was aborted when the J-41 ran over grassy ground beside the runway. The aircraft came to rest about 40 metres beside the runway without serious dmaage. Later the pilots were able to taxy back onto the runway under own power. There were no reports about any injuries.

On descent to Brisbane the pilots elected to divert to Coolangatta Airport instead because of adverse weather condition (thunderstorm, intense rain and high winds) at their destination. According to local airline sources, the aircraft took 'extensive damage' when it experienced multiple birdstrikes (geese) shortly after take-off from Kansas City at an altitude of about 4.000 feet. The right hand engine #2 spooled down as a result of bird ingestion. Fortunately the left hand engine produced enough thrust to perform a safe landing return at the airport about 10 minutes later.

Very shortly after take-off from Love Field Airport, an emergency chute suddenly deployed. The chute was ripped off by wind forces and fell to the ground together with its access door. Fortunately no one on the ground was injured and no property damage occured. The A319 safely returned to land at its point of origin. The aircraft was on a post maintenance flight after it was overhauled at the Associated Air Center.

On touchdown, or final moments of flight, the aircraft encountered multiple birdstrikes but was able to maintain full control. The landing was completed safely. Post landing inspection showed impact marks of dozens of birdstrikes (seagulls). 46 dead birds were counted on the runway. The aircraft sufered significant dmaage to the wing leding edges, nosecone, propeller blades, langing gear and pitot tubes.

On landing runway 27(A), the aircraft ran beyond the end of the runway coming to a stop after travelling over soft ground for about 150 metres. All passengers and crewmembers on board were able to evacuate without serious injuries.At the time of the accident visibility was reduced to about 2.5 kilometres and light rain was falling. Mumbai´s main runway 09/27 is reduced by half to about 1.700 metres on every tuesday from 12:00 until 18:00L. Only lighter planes up to the size of an ATR are allowed to operate during that time. This unique "Tuesday Restriction" is due to continue for another 20 weeks.

The aircraft crashed when it made an emergency return to Wilson Airport after the pilots reported engine troubles about 13 minutes after take-off. The cargo aircraft contacted the ground short of the runway (probably runway 32) where it smashed through the airport fence and ran into a ditch, breaking apart before coming to rest on the grass a few metres away from the runway. Both pilots received fatal injuries.

On take-off the aircraft apparently suffered a failure of its left engine a few moments after leaving the ground. The pilots made a forced landing on rough terrain outside the airport where the amphibious aircraft came to a stop close to a public road to the west of the airport. All occupants escaped without serious injuries. Aircraft is believed to be a hull loss.

A few minutes after take-off from Kennedy intl Airport, the aircraft lost a engine tail cone which fell into a residential area at Roosevelt northest of the airport. Fortunately, the falling part did not cause any injuries or property damage on the ground. The aircraft continued to Tokyo where it made a safe landing. The missing cone was discovered during a routine post flight inspection.

According to a report of the United Nations Office on Drugs and Crime (UNODC) the aircraft landed on a makeshift runway in the desert north of Gao. Local police and Interpol said there are indications that tons of illegal substances (cocaine) were offloaded and shipped away by road or smaller planes. Upon take-off, the aircraft may have suffered some kind of power loss forcing the pilots to make a landing in the nearby Sahara desert. On landing the aircraft was severely damaged, caught fire and burned to destruction. Presumingly all on board did survive the crash because no bodies were found inside the wreckage. According to different sourced the 727 was burnt after landing and there was never an intention for taking-off again. Malian and International police is investigating the origin of the flight and ownership of the aircraft. It is now widely belived the 727 is a 1978 built model recently registered in Guinea Bissao (J5-) last known operator was Senegal based Africa Air Assistance. It was ferried to Brazil in Mid-2009 and last seen at Fortaleza in August.

On take-off the aircraft lost a large part of the cowling of its left engine #1. The large part fell onto the runway 18/36. Twisted piece of the cowling remained attached and was bended upwards before the wing leading edge. The pilots continued the take-off but retrned for a safe landing at Istanbul Ataturk Airport.

On final approach to runway 27(A) at Mumbai International Airport the pilots aimed to a touchdown point short of the runway threshold coming close to ongoing construction workers who were forced to run out of the path of the incoming A320. The pilots abandoned the approach and initiated a go-around maneuvre. However, on the 2nd landing attempt the aircraft again aimed too short of the runway and made another go-around before a safe landing was finally achived in a 3rd landing attempt. At the time, runway 27 was shortened by 1.260 metres due to construction works. The shortened runway was properly marked and designated as runway 27(A). The incident is under investigation by Indian DGAC.

Very soon after take-off from Mirny Airport, the aircraft entered a steep bank angle to the right and crashed onto snow covered field. It hit a slag heap of an abandoned mine about 2 kilometers from the runway where it disintegrated on impact and burst into flames. There were no survivors among crew and passengers.  The aircraft was on a ferry flight repositioning empty to Irkutsk. At the time of the crash, local weather consisted of clear skies calm winds and temperatures of  minus 24 degrees celsius. Russian Ministry of interior grounded all other Il-76s in the fleet with immediate effect to allow technical inspections. 
NOTE: This accident represents the 4th hull loss of an IL-76 in 2009 (two of them were operated by Russian Ministry of Interior).
