aircraft went missing on a cargo flight near its destination Wamena. The pilot was
in radio contact with Wamena-ATC for a couple of minutes before contact
was lost. The remains of the aircraft were found 2 days later near their
destination. All three persons on board are presumed dead. The aircraft was carrying 1,6 tons of food supplies. Wamena
Airport is situated in a valley surrounded by 12.000 ft tall mountains.

On
landing runway 18R, both nosegear tires blew out or burst. In the rollout
process the nosegear suffered severe damage and both wheels seperated. Due
to ground friction the 737 came to a stop on the upper part of its strut
in a nosedown attitude. All on board were able to evacuate safely. The
runway 18R/36L remained closed for a longer time until the 737 and all its
debris was removed.

Embraer jet was cleared to taxy for runway 24R to cross the left runway
24L via taxyway Sierra (S). Few moments later the Dash 8 (DHC-8) received
its takeoff clearance from the same tower controller. The Embraer-jet
which pilot have also listened to the same ATC channel stopped short of
runway 24L to avoid a potential collision. At this time, the Dash 8 took
off some 1.500 ft away from them. The on-duty controller was a trainee
under supervision. Aviation sources revealed, the same trainee was
involved in another dangerous conflict situation on June 3rd in nearly
identical circumstances. |

On
landing runway 24 the pilots were unable to stop before the end resulting
in an overrun by a few meters. It is understood there were no serious injuries
among passengers or crew. Damage is considered to be repairable. The
nosegear received substantial damage when the aircraft came to a stop on
uneven terrain beside a public road. The accident promted the
closure of Caticlan Airport until the aircraft was removed. The single runway at
Caticlan is 950 meters in lengh.
NOTE: On January 11th 2009 another MA-60 of Zest Air was involved in an overrun incident at
Caticlan.

a vehicle crossing the active runway via taxyway "Mike" ahead of
them. However, the take-off was continued and the aircraft departed safely.
The vehicle, a Ford Explorer cleared the runway just moments before the
A320 has reached its position. The incident is under investigation. The

Soon
after touchdown the aircraft veered off the right side of the runway.
It came to rest in high grass beneath the runway. Its right wing took some
damage while some propeller blades broke off after they came into
contacted with an earth mound resulting in the seperation of the entire
propeller. All on board were able to evacuate quickly via the left side of
the aircraft. Video footage revelad that a dog crossed the runway
immediately ahead of the Do328. This may have caused the pilot to apply
the brakes very sharply until directional control was lost. The only
available runway at Tanah Merah in 1.050 metres in lengh.
NOTE: The incident marks the 2nd landing accident of an Express Air
Do328 after PK-TXL received significant damage on landing at Fak Fak in
November 2008 (check
our archives for further
information).

The
aircraft was damaged byond repair during a photo flight. According to
local media, the aircraft was observed flying at low altitude when a wing
dropped and hit the water or a reef before the pilots lost control. All on
board escaped without serious injuries mostly due to the fact the Twin
otter remained afloat for a considerable time. The accident is under
investigation.

| Shortly
after take-off the flying pilot was forced to take immediate action to
avoid a conflicting traffic which was rapidly approaching its flightpath.
The A320 stopped its climb and began a sharp descend combined with a
turn until the conflict was resolved and normal climb-out established
again. According to pilot statements the other traffic was most probably a
military aircraft. No injuries reported. |
